This plan is a discount medical provider organization (DMPO). It is NOT a health insurance policy and does not meet the minimum creditable coverage requirements under the Affordable Care Act. This plan provides discounts at certain health care providers for medical services.
Pricing & Savings
Discounts vary by provider
The range of discounts for medical or ancillary services provided under the plan will vary depending on the type of provider and the medical or ancillary service received. Family HealthCare International does not guarantee that any particular provider will provide a specific discount.
Your Role
Member Responsibilities
The plan does not make payments directly to the providers of medical services. Members are obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount medical provider organization.
Network Access
Provider Listing Policy
A list of all participating providers in your service area is available via our online provider search tool. This list is updated at least every 30 days to ensure accuracy.

Upon request, a printed list of all participating providers in your network will be mailed to you at no cost. Family HealthCare International ensures all providers listed have active contracts at the time of publication.
Account Rights
Cancellation & Refund Policy
Members have the right to cancel their membership at any time. If you cancel within the first 30 days of membership, you will receive a full refund of all periodic charges, less the nominal registration fee.
